Our Registered Nurse evaluates each client's needs and determines what service options would work best. The RN designs an individualized plan of care with the client and/or loved ones and leads the care team. The RN may also provide skilled services on an hourly basis depending on the client's needs.
- Wound care
- Medication management
- Coordinate client's care with physician and other health care providers
- Oversee/supervise the implementation of care plan by an LPN and/or nurse aides
- Evaluate the effectiveness/appropriateness of client's plan of care and make changes as necessary.
- Re-assess client's needs and progress towards goals; provide feedback to appropriate health care providers and family
