The Care Manager is a Registered Nurse with years of experience in managing the care of seniors or individuals with disabilities. She will:
- Assess the care management needs of the client
- Make recommendations for care and safety
- Coordinate efforts to get necessary care equipment and/or community services
- Communicate & coordinate with the physician and other healthcare providers regarding changes in condition, as well as manage prescription refills
- Coordinate with Medicare, other health insurance providers or long term care insurers.
- Set up appointments as requested, arrange transportation and arrange for follow-up services
- Be a phone call away for questions or assistance
- Visit client and family periodically to re-evaluate their needs
